Please note: Projects cannot be analyzed without a completed Chain of Custody/Laboratory Test Request Form. To avoid delays in the analysis of your project be sure the Chain of Custody/Laboratory Test Request Form corresponds with your analysis. Chain of Custody/Laboratory Test Request Form must be signed by personnel relinquishing samples. Analysis cannot be preformed without a signature.
